Indian TB cases 'can't be cured'

Tuberculosis which appears to be totally resistant to antibiotic treatment has been reported for the first time by Indian doctors.

Concern over drug-resistant strains of TB is growing, with similar 'incurable' TB emerging in Italy and Iran.

Doctors in Mumbai said 12 patients had a "totally drug resistant" form of the infection, and three have died.

The Indian Health Ministry is investigating the cases and has sent a team of doctors to Mumbai.

TB is one of the world's biggest killers, second only to HIV among infectious diseases.

Normally a patient with TB is given a six to nine month course of antibiotics to eradicate it.

However, new strains of the bacterium have developed which are increasingly resistant to the antibiotics most commonly used to treat it.

Partially drug-resistant TB can now found in countries across the world, and "multi-drug resistant" strains affect countries such as Russia and China.

'Serious threat'

The Indian reports will fuel concerns over the ability of doctors to contain the disease in years to come.

The doctors at the Hinduja National Hospital in Mumbai who discovered it said they had treated patients for up to two years with a battery of drugs, to no avail.

The patients came from slum areas of the city, they said, where close contact between people meant further spread was likely.

The American Centers for Disease Control (CDC) confirmed that the Indian strain did appear to be completely resistant.

Dr Kenneth Castro, director of its Division of Tuberculosis Elimination, said: "Anytime we see something like this, we better get on top of it before it becomes a more widespread problem."

Patients who do not finish their lengthy course of treatment also present the bacterium with the perfect environment for developing further resistance.

However, there have been repeated calls for the pharmaceutical industry to make more efforts to develop fresh antibiotics.

"What we're seeing is probably just the tip of the iceberg. We don't know how widespread this is because so few people are tested for drug resistance."

She said the high prevalence of TB in India, coupled with high population density within its cities, meant that the new type of TB could be a bigger problem than previous "totally resistant" strains.

"It's going to take a massive effort and change in political will to get to grips with this - not just from the Indian government but from everyone else. This is a global problem, not just an Indian one."

Indianapolis looks to future after firing Caldwell (AP)

INDIANAPOLIS ? Jim Irsay has big plans for the Colts' future. He just hasn't worked through the details yet.

Two weeks after overhauling the front office, Indianapolis took the next big step in its major housecleaning project Tuesday ? firing coach Jim Caldwell after three seasons.

"This (search) is something that's going to start immediately and I really think we're going to get a coach that's going to lead us going into the future, and I think it's a bright future," Irsay said Tuesday. "It's tough to change and go forward, we've had such excellence and greatness here over such a long period of time and that's what I expect us to do again."

Irsay is following the same plan he installed after the 1997 season. Back then, a season-ending loss on the road allowed the 3-13 Colts to clinch the No. 1 overall draft pick. The next day, Irsay fired the general manager and the head coach and eventually changed quarterbacks.

It could happen again with a little more deliberation.

The day after losing at Jacksonville to post their worst record (2-14) in two decades and wrap up this year's No. 1 pick, Irsay fired team vice chairman Bill Polian and his son, Chris, the Colts' general manager. Last week, Irsay hired 39-year-old Ryan Grigson as the replacement for the Polians.

Since then, Grigson and Irsay have been in almost constant meetings debating the future of the coaching staff.

On Monday, Caldwell and former Rams coach Steve Spagnuolo met in Indianapolis about the Colts' defensive coordinator job. Most took it as an indication Caldwell would be retained. That was still the conventional wisdom Tuesday.

Eventually, Irsay and Grigson proved the pundits wrong. Irsay said he told Caldwell of the decision at about 2 p.m., shortly before the team confirmed the firing.

"We've talked about where we want to be more balanced," Irsay said. "We want to be excellent on defense and more consistent, and I think that's something that we're looking at as part of the vision. I don't think the guy has to be offensive or defensive or anything like that. It's a heavy lifting process right now."

It's unclear where the Colts will turn next.

Yes, Grigson acknowledged, he has a short list of candidates. Not surprisingly, he didn't say who was on the list, which could include names such as Packers offensive coordinator Joe Philbin, Eagles offensive coordinator Mary Mornhinweg and perhaps Spagnuolo.

Neither Grigson nor Irsay provided a timeline for the hire.

"We want leadership. Leadership is important," Grigson said after making his first big decision in charge of an NFL team. "We want strong leadership, and we want someone who shares his vision in this new era of Colts football. We want the best man and the best leader and the man that gives us the best way to go."

One thing they did agree on: The future didn't include Caldwell

After winning his first 14 games, an NFL record for a rookie head coach, and becoming only the fifth first-year coach to take his team to the Super Bowl, Caldwell did a masterful job guiding the injury-plagued Colts through a thicket of injuries and back to the playoffs in 2010.

But those successes all came with Peyton Manning, who led the Colts to a league-record 115 regular-season wins in the previous decade and a record-tying nine straight playoff appearances.

This year, with Manning out the entire season, the Colts lost their first 13 games.

Among players and coaches, Caldwell was universally well-liked. The list included Manning, who won all four of his record-setting MVP Awards with Caldwell on Indy's staff, as well as perennial Pro Bowl center Jeff Saturday.

"I think coach Caldwell has done a very good job. He has gotten the most out of his players, and we play hard for him each and every week," Saturday said before the season finale. "We haven't necessarily played well, we've made mistakes and done things, but they have, oftentimes, been things that we've talked about in coaching meetings."

Outsiders often saw it another way.

Fans frequently complained about Caldwell's game management, and some critics referred to Caldwell as a "puppet" of the Polians.

Many never forgave Caldwell for pulling the plug on a perfect season in a Game 15 loss to the Jets in 2009 and pointed to the midseason firing of defensive coordinator Larry Coyer and the long delay in replacing Curtis Painter with Dan Orlvosky at quarterback as decisions that should have come much earlier.

Irsay and Grigson did not characterize Caldwell's 1,099-day tenure the same way as fans.

But with Grigson already searching for a new coach and presumably preparing to take Andrew Luck with the No. 1 pick, the questions now turn to Manning, who had his third neck surgery in 19 months on Sept. 8.

The Colts still are not saying much about Manning's recovery, and Grigson has not yet spoken with Manning, who is owed a $28 million bonus in early March.

"We're not even there with anything regarding Peyton Manning just yet," Grigson said. "We have to know about his medical stuff, first. There's so many things there."

Caldwell ended his Colts' tenure 26-22 overall with one AFC title, two division crowns and one bleak season that has left him unemployed just three years after replacing close friend Tony Dungy, the first black coach to hoist the Lombardi Trophy.

"This was a difficult decision," Irsay said. "I wanted to make sure we took all the time we needed to make sure it was the right decision. ... And just like 14 years, ago, it's a big change for the franchise and at the same time, there's players, coaches, many people on the staff that will go into the new day and get on with the work of 2012."

Google demos QR code Gmail access, claims something better in store

What's the big G up to here, then? It seems the Gmail team has been tinkering with a new secure method of accessing your precious email. Type your credentials into your phone, then scan a QR code in the browser to log in. It's ideal for public machines where typing your password might gift your credentials to any key-logging software. Sadly though, it seems the venture was just an experiment, with Google employee Dirk Balfanz confirming so on his Plus account. So, we might not be accessing our Gmail sans keyboard anytime soon, but with said staffer also teasing that his team are working on something "even better" who knows how we'll be logging on in the near future -- let's just hope it's not this.

Bomb material found in Thailand after terror warnings (Reuters)

BANGKOK (Reuters) ? Thai police discovered bomb-making material after the detention of a Lebanese man suspected of planning an attack, but the prime minister insisted Monday that everything was under control.

Authorities beefed up security in parts of the capital and other areas popular with tourists after the United States and Israel warned Friday of a possible terrorist attack.

Police detained a Lebanese man reportedly carrying a Swedish passport. Officials said he had links with Hezbollah, a Shi'ite Islamist group in Lebanon backed by Syria and Iran that is on the U.S. blacklist of foreign terrorist organizations.

National police chief Priewpan Damapong told reporters the suspect, named as Atris Hussein, had given police an address where bomb-making material was being kept.

Officers discovered large amount of substances that could be used to make explosives in a building in Samut Sakhon, southwest of Bangkok, including 4,380 kg of urea and 10 gallons of liquid ammonium nitrate.

Priewpan said the suspect maintained that his group had not planned an attack in Thailand but intended to transport the substances to a third country. The officer declined to give the destination.

Asked about the discovery, Prime Minister Yingluck Shinawatra told reporters: "I have been informed. I would like to ask people not to panic. We are currently in control of the situation."

Defense Minister Yuthasak Sasiprapha told reporters in the northern city of Chiang Mai Sunday that Thailand was not the target, although officials have also said that areas of Bangkok frequented by Westerners and Israelis could be hit.

Yuthasak said that a second suspect had managed to leave the country. Police said the detained suspect had not yet been charged with any offence.

Thai officials have seemed irritated by travel advisories issued by the U.S. and Israeli governments, followed by several more since Friday, and Foreign Minister Surapong Towijakchaikul said diplomats from countries that had issued warnings would meet with him for an explanation Monday.

Tourism is a big money-earner for Thailand and ministers are keen not to deter travelers, especially after the hit to tourism from severe flooding in 2011 and political unrest in 2010.

Heather Locklear taken to Calif. hospital (AP)

THOUSAND OAKS, Calif. ? Heather Locklear was taken to a Southern California hospital for precautionary reasons Thursday after an emergency call was made from her home, authorities said.

Paramedics and sheriff's deputies responded Thursday afternoon to Locklear's home in Westlake Village, which is 35 miles northwest of Los Angeles.

Locklear was taken to Los Robles Hospital & Medical Center in Thousand Oaks, where a spokeswoman told KCAL-TV the actress was stable and her parents were by her side.

"Her parents wanted everybody to know that she's doing well, she's fine, she's not in any danger, she's healthy," hospital spokeswoman Kris Carraway-Bowman told the station.

Ventura County sheriff's Capt. Mike Aranda said he did not know Locklear's condition but deputies were not investigating.

Locklear has been hospitalized several times over the years. In 2009, she pleaded no contest to reckless driving after being arrested on suspicion of driving under the influence of prescription medication.

The 50-year-old actress's publicists did not return messages seeking comment.

Locklear and "Melrose Place" co-star Jack Wagner recently ended their engagement. She was previously married to Bon Jovi guitarist Richie Sambora, and they have a daughter together.

French fast-food chain offers 'Star Wars' burgers

French fans of "Star Wars" will soon be able to show their preference for either the noble Jedi or Darth Vader's dark side simply by ordering a burger.

Quick, a European fast-food chain that's similar to McDonald's, is rolling out a promotion?from?Jan. 31-March 5,?tying in to the Feb. 8?release of "Star Wars Episode 1: The Phantom Menace" in 3-D.

First, diners can choose between a Darth Maul-supported Dark Burger (made hot with a pepper-sprinkled bun, pepper sauce and pepper cheese)?and a Yoda-supported Jedi Burger (topped with Mozzarella cheese and spicy mustard).

But the real blockbuster comes only from March 2-5, when Quick diners can order the "Dark Vador" (their spelling)?burger, which features a bun dyed as black as Vader's heart (the chain says it uses a "vegetable coloring agent" to get the black bun). Quick is calling this the "first black coloured burger ever created." Red pepper slices and cheese top the burger patty.

That's Mozzarella cheese on the Jedi burger. Looks like marshmallows to us.

The chain will also offer fried cheese snacks shaped like stars and a special ice-cream treat, as well as Amidala and Anakin toys.

It's not the first time Quick has served up creative burgers. In 2011, the chain offered a burger called "Le Double Mix," which featured a split bun and used different?sauces on the burger's two halves.

Sadly for American "Star Wars" fans, none of the chain's 495 restaurants are on our side of the pond. These are not the burgers we're looking for.

Amid crisis, Pakistan president travels to Dubai (AP)

ISLAMABAD ? Pakistani President Asif Ali Zardari left for Dubai Thursday on a private visit, officials said, leaving behind a widening rift between the powerful armed forces and the civilian government.

Early last month, Zardari went to Dubai for medical treatment, and it triggered speculation the leader was pushed out by the army or was fleeing a potential coup.

The officials said that the president would attend a wedding in Dubai and would be back in Pakistan on Friday morning, in a trip unconnected to the current crisis. They didn't give their names because they were not authorized to release the information.

As Zardari left, Pakistan's military chief was meeting top commanders, media reports and a military officer said, fueling speculation about the army's next move.

While most analysts say army chief Gen. Ashfaq Pervez Kayani has little appetite for a coup, they say the generals may be happy to allow the Supreme Court to dismiss the government by "constitutional means."

On Wednesday, the prime minister fired the defense secretary and the military issued an unusual statement warning of "grievous consequences", as the fallout from a scandal centered on a memo written to Washington asking for its help in reining in the generals widened.

The Supreme Court, seen as hostile to President Asif Ali Zardari and an ally of the army, is investigating that affair and a second one linked to past corruption cases against the president. Both could potentially be used as a pretext to oust the current civilian leadership.

Pakistan's army has ruled the country for much of its six-decade existence, and still sees itself as the rightful custodian of the country's interests. No civilian government has ever completed its term in office.

The Zardari government, which was democratically elected in 2008, is determined to see out its term.

General elections are scheduled for next year, but could well take place sooner.

The government has been widely criticized for ineptness, poor or ineffectual governance, and alleged corruption.

Still, domestic and international proponents of democracy say it should be able to complete its term, and elections should decide the country's next leaders. They note successive military coups in Pakistan are a main cause of the country's current malaise.

Spotify to End Unlimited Free Streaming for Some Users Next Week (Mashable)

Spotify's unlimited free streaming and sharing service was music to American ears when it launched in July 2011. Now comes a dissonant note: ten-hour monthly caps will be implemented on all free accounts when the six-month promotional period comes to an end for early adopters next week.

"All new Spotify users, both in the U.S. and in all other countries in which Spotify is available, are able to enjoy an unlimited free service for their first six months. As we have previously stated, after a Spotify user has enjoyed free unlimited listening for six months, some changes to the free service will come into effect," the company said in a statement.

Spotify has garnered 10 million users worldwide and a base of more than 2.5 million paid subscribers since it was developed in Sweden in 2008 and launched in various parts of Europe.

Free versions of the desktop app were available through invites only. An unlimited service was offered at $4.99 per month and the premium service was available for $9.99 per month. The Spotify Unlimited and Spotify Premium services were available free of ads and with unlimited music.

Spotify Premium ($9.99) offers offline playlists, unlimited ad-free music and enhanced sound.

The number of users exploded after users were able to sign in and download the Spotify music software via Facebook, avoiding the invite-only process. It was never meant to be a free service that would last forever. Unlimited free streaming was set to expire for U.S. users after they used the app for six months of streaming, as it had with the launch of the app in the UK in 2008.

Spotify's CEO Daniel Ek, while announcing the Facebook partnership at last year's f8 developer conference , said that more people will pay for music -- staying away from pirated stuff -- with access to the great variety of high-quality music that Spotify provides.

Says the company's Terms and Conditions of Use: "Spotify Service can be accessed as an ad-supported free-to-the-user service having no monthly cap on listening hours or a cap on number of plays of a unique track during the first 6 months following creation of your Spotify account, but thereafter a cap of 10 listening hours per month and a cap of 5 plays per unique track (the 'Free Service')."

Spotify's popularity stems from the fact that you can stream any song or album, drawn from a wide selection of music. Playing a song on the Spotify player will automatically alert your Facebook friends, allowing them to learn what you like and discover new music. Users may also drag songs into friends' profiles to share tunes.

Users can also take Spotify on the go -- the free mobile app is available on iPhone, Android, Symbian, Windows Phone, Palm and Blackberry, which allows streaming over WiFi (premium service), offline playlists (premium service), access to your Spotify account and syncing your own music into Spotify from your phone.

GOP Candidates from New Hampshire Eyes (ContributorNetwork)

COMMENTARY | As the primary approaches my decision has been influenced by a number of different variables. The No. 1 method that I am influenced is by the advertising either on television or YouTube. Mitt Romney and Ron Paul have taken over YouTube with their campaigning ads. Before every clip I go to see I hear Romney or Paul talking to me about what it means to be an American.

Romney seems incredibly patriotic and is from New England, while Paul is always talking about how he predicted the economic collapse that happened. I would say my vote is a toss-up between these two men just based on their background and charisma that they possess.

Romney seems to have everything that is desired in a president. But after all his campaigning he seems to sometimes let his tongue slip and say something that I don't agree with and comes off as immature, such as when he is talking about Barack Obama and will not leave the birth certificate situation alone.

He has been president for nearly three years now and I believe Romney may need to learn to control what he says. If not, it could be detrimental for America. The role of the president is largely to be the face and voice of America. Romney has the looks and charisma, but he needs to learn to control what he says, because if he lets something slip to the wrong person the consequences could be disastrous.

Paul has presented an idea that I am completely at odds with, his idea that there should be no separation between church and state. There is an age gap between these two men and that may be why some of their policies are so different.

A lot of the ideas that one carries into adulthood are brought from the influential stages of childhood. Paul was growing up with a different situation than Romney. Paul being a Baptist Christian, like much of America, can incorporate that into his policy. However Romney on the other hand is a Mormon and would have a hard time putting a Mormon policy into his run for president. Romney seems to have that home-town hero affect on the New Hampshire voters and also has the patriotism and charisma that is sought after in an American president.

THE NEWS: Swiss National Bank Chairman Philipp Hildebrand resigned Monday, bowing to a public uproar over private currency deals just as a Swiss parliamentary committee was preparing to grill him.

THE BACKGROUND: Reports say Hildebrand and his wife, a former currency trader, netted tens of thousands of dollars in profits last year from dollar swaps while the banker was working to lower the value of the soaring Swiss franc.

THE EXPLANATION: Hildebrand says he broke no rules but added that a central bank chairman must be viewed as beyond reproach and that he could not prove his wife made the transactions without his consent.

Billionaire gives $5 million to pro-Gingrich group (AP)

MANCHESTER, N.H. ? A Las Vegas billionaire with ties to Newt Gingrich has given $5 million to an independent group backing the former House Speaker's presidential bid.

A person familiar with the situation said casino mogul Sheldon Adelson made the contribution Friday to Winning Our Future, a pro-Gingrich super PAC. The person said Adelson would spend heavily to assist whichever candidate wins the Republican nomination.

Adelson is a longtime supporter of GOP candidates and has been a friend and backer of Gingrich for many years.

Gingrich's campaign has struggled since a super PAC supporting Mitt Romney spent $3 million on ads attacking him in Iowa.

The Washington Post first reported the $5 million contribution. Politico reported last month that Adelson was prepared to spend $20 million to help Gingrich.

Rick Matasar: 'The Ron Paul of Legal Education' or 'My Hero'?

As administrators and faculty members from the nation?s law schools gathered here today, there was a general sense among those assembled that legal education is facing challenges. But they found little agreement on what exactly those challenges are, let alone what will be necessary to solve them.

The majority of sessions Thursday were dedicated to subjects other than the turmoil that has swept through the law school world in the past year. But in a workshop dedicated to the future of the legal profession and legal education, much time was spent discussing the new reality for law school graduates and what, if anything, law schools can do -- even as it largely elided?the more controversial aspects of the situation, such as charges that law schools have deceived their students by reporting misleading employment data.

?It?s an extraordinarily volatile, and in some ways depressing, demoralizing environment in which to work,? said James G. Leipold, executive director of the National Association for Law Placement. ...

Much of the conversation focused on how to better prepare students for?the job market, whether through increased clinical study, partnering with law firms that want to hire graduates trained in a specific field or method, or other curricular changes that acknowledge that the traditional path through a big law firm is closed to more graduates than in the past. Other presenters focused on cutting the prices students pay so they would have to borrow less money over all. And some, including the president of the ABA, delivered a full-throated defense of American legal education, which they said may not be perfect but is still valuable.

Most strikingly, some law professors, administrators and lawyers took a position more frequently put forward by faculty in the liberal arts: that even if students with law degrees do not practice law (and an increasing number of students are indeed taking jobs that do not require a J.D.), a legal education provides a strong foundation for work in a variety of fields through encouraging writing and critical thinking.

?It?s law graduates who don?t practice law who are often most complimentary about their legal education and the analytic skills they received,? said Judith Areen, a professor at the Georgetown University Law Center.

The most forceful argument for reform came from Richard Matasar, dean of the New York Law School, who has argued for years that law schools need to change. (Areen called him the ?Ron Paul of legal education.?) ?If we do not choose to be the agents of our own change, there will be others who are incented to think about making those changes happen,?and they will not be educators,? At the other end of the spectrum was the president of the ABA, William Robinson III, who defended American legal education as the finest in the world. ?So many who never went?to law school want to talk about law school as a trade school,? Robinson said. ?It is not. It is a school of higher learning? -- a statement that drew spontaneous applause.

National Law Journal, AALS Hears Words of Caution from Departing Dean:

[F]ormer New York Law School Dean Rick Matasar ...delivered a cautionary speech during the annual meeting of the AALS in Washington on Jan. 5. "We know there are storm clouds on the horizon," Matasar said, as he ruminated about the poor job prospects facing students, the growing debt load of graduates and the possibility that outside regulators will force changes upon law schools. ...

He has long argued that real change would be painful for administrators and professors alike. His views that law schools need the flexibility to take different approaches toward education, such as having faculties that do not enjoy tenure or produce scholarship, have not always been popular. Fellow speaker Judith Areen, a professor at Georgetown University Law Center, joked: "There are some who call Rick Matasar the Ron Paul of legal education." ...

In his parting words, Matasar urged legal educators to think about five new approaches:

  • Diversifying to find money sources and build services beyond the traditional J.D. program
  • Stratifying the marketplace to allow schools serve different functions and fill different niches
  • Collaborating across schools to stretch resources by, for example, sharing faculty members
  • Enrolling students who lack B.A. degrees, as legal educators in other countries do
  • Employing new approaches, such as games and computer applications, that help students learn the law
Matasar's message won high praise from University of Louisville Louis D. Brandeis School of Law Dean Jim Chen. "Once and for all, Rick Matasar, you are my hero," Chen said. "No one wants to pay for something that doesn't pay off."

First, if law schools were indeed so worthwhile they wouldn't have to publish fraudulent post graduation employment data to attract students. Second, the basic skills required for legal work are learned in 1L and *maybe* the first half of 2L. Period. The rest of 2L and all of 3L are far less useful should be replaced with a medical-school-like residency in a law firm or similar environment.

I don't think anyone is faulting legal education for lack of perfection, just being overpriced and oversold. I don't think the ABA is paying attention.

In the olden days, some (many?) aspiring lawyers would eschew law school altogether and "read the law." Law is just one area in which I'd like to see less institutionalized and more independent study. I imagine law school administrators would not agree.

Sprint encroaches on AT&T markets for first LTE upgrades: Atlanta, Dallas, Houston and San Antonio

We knew good and well Sprint was converting its WiMAX network to LTE, but up until now, we had no idea who would get first dibs. Sprint's own Dan Hesse just cleared up a quip that he gave to us back in July of 2011, noting that Atlanta, Dallas, Houston and San Antonio would be first to get "4G LTE and 3G network upgrades by mid-2012," just in time for the first Sprint LTE phones to take advantage. If you're thinking those cities sound familiar, you're right; AT&T Mobility is headquartered in the ATL, and all three of those Texas communities were high on AT&T's list of launch markets, too. Here's hoping we hear of a few more markets in the run-up to Mobile World Congress in February....

Debate over who needs a thyroid check in pregnancy

WASHINGTON (AP) ? Check-ups during pregnancy tend to focus around the waist. But there's growing debate about which mothers-to-be should have a gland in their neck tested, too.

Numerous studies since 1999 have found that an underactive thyroid can raise a woman's risk of miscarriage, premature birth, or a lower IQ for her baby ? even if it's so mildly sluggish that she feels no symptoms.

The problem: While serious cases are treated with a hormone pill, so far there's little evidence that treating the milder cases makes a difference. So guidelines about who should be tested vary widely.

Now a peek at prenatal testing from one of the country's largest medical labs suggests that nearly a quarter of pregnant women are getting the simple thyroid blood test regardless of whether they have symptoms.

Researchers at Quest Diagnostics examined records for half a million pregnant women. Of those who got tested, a higher-than-expected number ? 15 percent ? had an underactive thyroid. That's five-fold higher than some previous estimates, partly because the way in which the condition is diagnosed has changed recently, says the study published by the Journal of Clinical Endocrinology & Metabolism.

The vast majority of those women were in the gray zone, with milder cases where no one knows for sure if a diagnosis helps or wastes money on testing and thyroid medication.

The finding adds pressure for science to settle this long-running controversy.

"We still don't have perfect answers," says Dr. Elizabeth Pearce, a well-known endocrinologist at Boston Medical Center, where a recent survey found widespread prenatal thyroid testing. But, "if it's my patient in my office, or it's me or my family member, I'm going to treat every time."

Obstetricians seem more wary.

"There are studies on both sides of the fence," says Dr. Dena Goffman of New York's Montefiore Medical Center, which tests only women at high risk. "If you don't know what to do with the results, you probably shouldn't order the test," she said.

The unassuming thyroid ? a small bow tie-shaped gland nestled in the front of the neck ? plays a big role in good health for everyone. It produces hormones that regulate metabolism and can affect almost every type of tissue in the body. About 20 million Americans are estimated to have a malfunctioning thyroid that, if serious enough, can contribute to heart disease, bone-thinning osteoporosis and infertility.

An overactive thyroid, called hyperthyroidism, speeds up bodily functions, causing such symptoms as weight loss, nervousness, anxiety and increased heart rate and vision problems.

Much more common is an underactive thyroid, called hypothyroidism. It slows body functions, causing such problems as fatigue, weight gain, depression, constipation and dry skin. It even can contribute to high cholesterol, according to the National Institutes of Health.

Thyroid problems increase with age, but they affect far more women than men ? and pregnancy puts extra stress on the gland.

Having enough thyroid hormones is important for fetal brain development, especially during the first trimester, when the fetus depends solely on the mother for them. The hormones also play a role in avoiding miscarriage or premature birth.

Mothers also may harbor immune system cells called antibodies that subtly attack the gland and likewise are linked to miscarriage and prematurity. Italian researchers found that treating those women lowered their risk of encountering this problem.

There is broad agreement that women with overt hypothyroidism ? a seriously underactive gland ? should be treated, most likely given a once-a-day hormone pill long known to be safe in pregnancy. But it takes blood testing to diagnose overt disease because even those women don't always report the vague symptoms.

Those $25 blood tests are sure to uncover women with mild hypothyroidism, too, the people in the so-called gray zone. Some research has raised questions about whether mild cases really pose a pregnancy risk, and preliminary results from a large British study recently found no overall IQ benefit to the resulting children if their mothers had been treated.

But the damage might already have been done by the time treatment began late in the first trimester, notes Boston's Pearce. In the U.S., doctors are anxiously awaiting a similar National Institutes of Health study; results aren't expected until 2015.

What's the advice until then? The American College of Obstetricians and Gynecologists recommends testing only pregnant women who have thyroid symptoms, have had previous thyroid problems, or have similar autoimmune diseases such as Type 1 diabetes ? those considered at risk for overt disease.

The American Thyroid Association goes further. Last summer it advised also testing all pregnant women age 30 and older, and those with enlarged thyroids, previous pregnancy problems or those who are obese, says Pearce, who co-authored the guidelines.

Most guidelines cite the lack of evidence for treating mild cases. The thyroid association does urge treatment if those women harbor the worrisome antibodies.

A final tip: Pregnant women should check that their prenatal vitamins contain iodine, important for proper thyroid function, Pearce says. Not all do. Most Americans get enough iodine from dairy products, bread, seafood and iodized table salt. But women need extra during pregnancy.

LOS ANGELES ? Russell Brand and Katy Perry are getting a divorce, the British comedian told The Associated Press Friday.

"Sadly, Katy and I are ending our marriage. I'll always adore her and I know we'll remain friends," said Brand, 36, offering no other details.

He and the 27-year-old pop singer were married in October 2010 at a resort inside a tiger reserve in India.

Brand's recent film credits include "Arthur," "Hop" and "Get Him to the Greek." He is among the ensemble starring alongside Tom Cruise in "Rock of Ages," set for release next year.

Perry's run of No. 1 singles earned her the distinction of becoming MTV's first artist of the year earlier this month. She hosted "Saturday Night Live" on Dec. 10 and gave no indication the couple was in trouble.

The couple got engaged in January 2010 after meeting at the 2009 MTV Video Music Awards, where he hosted and she performed.

He was effusive about his bride while promoting projects earlier this year, saying marrying Perry has "given me much more strength in what I do."

"For a long while, what I do professionally was all that mattered to me really," he said in March. "Now I think, well, whatever I do, I'll just go back to her, and that's incredibly comforting."

He cited irreconcilable differences in papers filed Friday in Los Angeles Superior Court.

A representative for Perry did not immediately return a call seeking comment.
